PRIMARY PLAN ($350)
Includes:
- Initial Comprehensive Exam and Full set of X-rays
- Two professional dental cleanings ( Adult Prophylaxis)
- Two routine exams
- Unlimited x-rays
- Fluoride treatment once per year
- One emergency exam per year (if needed)
- 10% off restorative treatment
- A minimum savings of $155 annually
ADVANCED PLAN ($450)
Includes:
- Three professional dental cleanings ( Adult Prophylaxis)
- Two routine exams
- Unlimited x-rays
- Fluoride treatment once per year
- One emergency exam per year (if needed)
- 10% off restorative treatment
- A minimum savings of $255 annually
Limitations, Rules, and Exclusions
- Valid only at Deep River Family & Cosmetic Dentistry.
- Member fees must be paid at registration.
- No refunds can be given after registration.
- Membership is for 12 consecutive months
- Cannot be combined with insurance plans. Obtaining any insurance policy while under membership will result in loss of membership.
- Failure to show or cancel an appointment in less than 48 hours for a scheduled cleaning appointment will result in a cancelation fee.
- Scaling and root planing are not considered adult prophylaxis and will be charged at the regular fee with a 20% discount.
